[stgt] [PATCH 0/2] Fix Windows iscsi initiator's formatting failures

I have verified with a W2K8 host that without Alexanders two patches
formatting with NTFS will fail with an error.
But after I apply Alexanders two patches I can successfully format a
LUN with NTFS.
